My boyfriend goes into DKA frequently no matter how closely we monitor his glucose levels, testing every couple hours. Anything we can do different?

Type 1 on 38u of lantus in the am and sliding scale of regular. Has gastroparesis that makes it difficult to eat all the time, and to judge how quickly his body is processing the carbs he is eating.

Good morning LoAnn; Thank you for consulting with dLife. Great job in being a super support person to your boyfriend. The occurance of DKA is usually due to deficiency in insulin. Without enough insulin, the body begins to breaks down fat as an alternate fuel. In turn, this process produces toxic acids in the bloodstream called ketones, eventually leading to diabetic keto acidosis if untreated. This develops because there is no insulin or not enough insulin. Interruption of an adequate supply of insulin can lead to diabetic keto-acidosis, (DKA).

When in doubt, check the urine for ketones/keto-strips are available at over-the counter in the pharmacy section. Technology available, such as the pump or the glu-watch is equipped with alerts, warnings and alarms of impending lows. Learn the warning signs of diabetic keto acidosis and know when to seek emergency care, DKA can develop within 24 hours. Contact your doctor immediately if: You're vomiting and unable to tolerate any food or liquid Your blood sugar level is higher than your target range and doesn't respond to home treatment. Your urine ketone level is moderate to high. Your blood sugar level is consistently higher than 300 milligrams per deciliter Be proactive to signs/symptoms;

  • Frequent urination
  • Nausea and vomiting
  • Abdominal pain
  • Loss of appetite
  • Weakness or fatigue
  • Shortness of breath
