"Would it be advisable to take an extra dose of 70/30 insulin if your work schedule has you taking two doses 12-14 hours apart?"

Currently my work schedule has me getting up at 5:00am and taking 50 units of 70/30 insulin along with 1000mg of Metformin. I get home from work around 7:00pm and take another 50 units of 70/30 and another 1000mg of Metformin. I am finding that by noon my sugars are going high and staying high after lunch. They are averaging about 250-300 until I get home. Would it be good to take an extra dose of insulin to keep my sugars under control?

Dear kristishorten, If I had your work schedule I would not be taking 70/30 insulin. The actions of the two kinds of insulin in this mixture are not aligning up with your mealtimes. You might talk to your physician about ordered a 24 hour insulin and then use a short-acting insulin at mealtimes. Usually the 24 hour insulin is about 50% of the total units of insulin you need per day and the other 50% is the mealtime insulin shots. 70/30 works well when someone eats 3 meals per day and the meals are about 4-5 hours apart. Your schedule does not meet this criteria.
