Hello Nina2003: Thanks for asking dLife and great job with the weight loss and monitoring of the blood glucose levels. (70#s in 6 months-you must be proud of such a super accomplishment!!!

The recommendation for the use of Metformin, it should be taken with meals, as you do, its primary function is to reduce liver glucose production, as well as stimulate glucose uptake in insulin sensitive tissues. It supresses excess liver glucose production and increases sensitivity of all cells/muscles to YOUR circulating insulin.

I do believe, the best time to assess glycemic max excursions, would be 2 hours after a meal is consumed. Time from the start of the meal, say 6 pm, your meal averages 20-30 minutes to be totally consumed, (your savor the taste and chew your food really well), by 8 pm the total impact of the carbohydrates would peak and are beginning to decline because your circulating insulin-being super sensitized by Metformin, is opening the cells to allow glucose into the cell and continue on with metabolism, (energy production occurs inside the cell).

Monitoring is very important, giving instant information regarding how effective all your efforts are impacting the glucose levels, (stalling off any potential of adverse complications :)

Preferable blood glucose levels should not exceed (strict control) 140 mg/dl per the AACE, or 170-80 mg/dl per American Diabetes Association.

If you are testing 2 times per day, I would suggest changing times as to reflect your waking 10-12 day, i.e. one day, before breakfast and then 2 hours after lunch, the next day, before lunch and before dinner, or by the end of the week you can plot out the log book results and see at what point there are highs and lows, and then link back to diet or exercises changes. Hope that makes sense. Be your best. Regards Sue
