"My health care consultant has raised the possiblity of taking a 2nd shot of Lantus...I currently take 26 units at night "

she has proposes taking an additional smaller dose in the AM as my pre dinner readings are still in the 180 to 225 range..everything i have read about Lantus indicates a 1 shot a day insulin....My fasting numbers in AM have been less than 100 even in the 60's and lunch is around 100 to 120.... is this accurate??

Dear Barbara: It sounds like you have a wise Primary Care Provider (PCP)!!! Yes, Lantus is FREQUENTLY split into two doses. Everyone responds differently to insulin. Some absorb quickly, others slower. Some find insulin stays in their system for many hours, others; very few. Anytime the Lantus does not appear to be "lasting" the full 24 hours, it's a good idea to split the dose. We find this to be particularly common in people who take relatively small doses of Lantus (which you do). At this point in time, it appears that your PCP is striving to normalize all your BG levels. You do not mention taking any mealtime insulin. This would be the alternative to splitting Lantus; taking a rapid acting (mealtime) insulin before lunch. For ease and convenience, she suggested extra a.m. Lantus. It sounds like the best first solution to me. Hang in there!

I was up to 62 units a day but it was not lasting the full 24 hours. I chose to split my dose, with my doctor's approval. 31 units twice a day along with the Humolog at meals works great for me and my doc is happy also

I have read that although Lantus may typically have a 24 hour, fairly even effect, for some patients this is not the case. For me, I found that Lantus tends to last about 18 hours with a much stronger effect toward hours 12-15. I got *MUCH* better coverage by splitting my Lantus dose and taking it twice a day. I also tried Levamir which did work better over the 24hr period, but Lantus was covered by my insurance at a lower co--pay.
