It is always frightening when blood sugars are very high. However, taking Lantus at 11 PM and not at 9 PM would not be the cause of these high levels. Although we recommend taking Lantus about the same time every day, within an hour earlier or later, a two hour window would not cause a significant problem. There are many reasons why blood sugar can be high. I see that his mother was trying to bring his blood sugar down by using a meal ratio that would give him more insulin. Does your son have a "correction factor?" When blood sugar is high at meal time, we can add insulin to his meal time dose by using the same meal time ratio and then adding a correction dose in addition. A typical correction factor is 1:50 meaning that one unit will lower the blood sugar 50 points. If the blood sugar goal is 150 and his blood sugar is 450, then you would give an additional 6 units to lower the blood sugar to goal. Sometimes providers will suggest that you add a correction dose in between meals when blood sugars are very high. Most importantly, you do need to determine why his blood sugar was so high. Often a sudden rise in blood sugar can be a sign that he is getting sick with a cold or flu. Ideally you should have a plan in place for when blood sugar is high. This plan should be written down and agreed upon by both parents and the provider. You need to know when you should call the provider to report high blood sugar as well. Do you test your son's urine for ketones when the blood sugar is high? This is another item that needs to be on your plan. High blood sugar can lead to Ketoacidosis which can be potentially life threatening so dealing with high blood sugar in an efficient and effective manner is important. Frequent blood sugar testing when the blood sugar is high is very important. Have your son drink plenty of sugar free beverages because high blood sugar can cause dehydration. Several problems exist. The correction factor you use is different for everyone with TYPE 1.If i used 1/50 my BGS would be the same. This is something you and your care team should work out You might think about testing an hour before meals.I would cover the high bgsbefore i ate The other problem is your carbcount ability HEALTH CARE TEAM !!!!!!!!!!!!!!!!!!!!!!!1
