"My last GFR was 28. I can't find a diet plan and ins. doesn't cover a nutritionist. I'm 5'8" and weigh 149. I am 49 yers old can you help me"

Type 1 diabetes since 1972 Anemic, High BP, Neuropathy of feet, legs and hands, Gastrpharesis (spelling?) Depression, anxiety disorder. Recurrent ulcers right foot.

Hello SharonFarris: Yes, we can definitely help, but, YOU need to act-The foot ulcer needs attention immediately, infection will mess with your readings and those high glucose levels are hurting you kidneys!
With kidney involvement and other neuropathies, I would suggest doing a nutrient analysis on www.fitday.com, at this site you can establish your baseline food intake. You enter your entire day-days intake and it will analyze for totals: carbs, protein, fat, nutrients and water.
  • Compare with the following general meal plan: 1600 total calories, 50-60 grams protein/day, distributed carbs, proteins and fat evenly throughout the day, small snack in mid-afternoon and bedtime snack.
  • Or 40 gms carb-Brk,
  • 50-60gms carb-lunch&dinner,
  • 20-30 gms carb per snack. (Considering, the gastropharesis, small frequent meals may be more appropriate, based on your tolerance.

    Carbohydrate fuels the blood glucose 100%, proteins ~50% and fat~20%. So, the first 2 hours after a meal tell me the carb intake glycemic highs, at 3-4 hours that reflects the protein from the meal. Lastly, the fat influences. I like to keep the meal plan and calculating choices simple:

  • the average small piece of fruit, slice of bread, 1/2c. pasta, potatoes, corn, peas, legumes, 1 c. milk(any fat content) is about 15 grams carbs
  • low carb veggies: green bean, tomatoes, broccoli, cabbage, carrots is about 5 grams for 1/2 cup cooked or 1 cup raw.
  • There is no substitute for reading labels, as soon as you think you know how many carbs, the manufacture changes the recipe!
  • The proteins: beef, chicken, fish, if it's a solid piece, 1 oz.=your thumb size and has about 7 gms protein. If its ground/flaky/consistency, 1/2 cup=3-4 oz. or 1/2 the size of your fist. (You have built in measuring tools).

    At a GFR of 28, this is overt nephropathy indicating both protein and sodium restrictions. In addition to the protein restriction listed aim for approx. 2-3,000mg/Na+ per day.
