"I'm a type 2 diabetic on insulin and have been diagnosed with cataracts. How will my diabetes affect the surgery and healing process?"

I'm on Levemir flex pen - 36 U at bedtime and Novolog flex pen on a sliding scale twice a day. Was diagnosed 6 months ago & my blood sugars seem to be under control & then they go haywire and then come under control again. Metformin sent me into the bathroom 6-8 times a day with diarrhea. My vision is being strongly affected by the cataracts, particularly at night.

Hello Canaandog1; Yes and yes. The most important factor is the control issue, get the blood glucose under control. The stress,(surgery, injury, sometimes good or bad), these stress ors trigger counter regulatory hormones, these hormones then trigger the release of stored sugar in the liver. Possibly prior to surgery the doctor has an insulin drip, or makes other provisions for the glucose raising effects, or possible you may need just a little extra monitoring and t wicking of insulin. As always, keeping the levels under control stall off any of those possible complication. A is for A1C, previously known as hemoglobin A1C—a test that reflects average blood glucose over the last 3 months. The goal for most people with diabetes is <7. An A1C of 7 corresponds to an average blood glucose level of 150 mg/dL. B is for blood pressure. The goal for most people with diabetes is <130/80 mm Hg. C is for cholesterol. The goal for people with diabetes is an LDL level of <100 mg/dL, an HDL level of >40 mg/dl in men and >50 in women, and triglycerides level of <150 Source: American Diabetes Association Standards of Medical Care, Diabetes Care 29 (Suppl.1): S4-S42, 2006 (27).
