"Maintaining an erection has not been my problem, but I avoid sex because after ejaculation I have painful symptoms almost like a UTI. What can I do?"

32 year old male, Type I diabetic, diagnosed at 17 years old. The pain and symptoms only occur immediately after sex and only last 30 minutes to an hour. It is similar to UTI pain, discomfort and an urge to urinate. No problem getting/maintaining an erection. Have been screened for STDs and everything is negative. What causes this? I have heard about retrograde ejaculation and think that might be the problem but I really don't want to have surgery. Are there other options?

Dear Katty, First of all thank you for asking a difficult question. It is wonderful that you are able to be sexually active. At your age there is no reason not to be. Type 1 diabetes should not effect your sexuality, especially if your blood sugars are well maintained. From your question it seems that this pain that you are experiencing is temporary and post intercourse. I also understand that you have been tested negative for any urinary infection. Therefore, the next step would be to ask your endocriologist about the situation and see what they suggest. The specialist who would deal with this problem in my experience is a urologist.
