"I was recently told I have a hole in both of my optic nerves and the beginning or Glaucoma. (I have had type 1 for 34 years). I cannot find any information on this combination and I am not sure if it is 1 or 2 different problems? I am now using drops and I have a follow up appointment 7/18. Any information would be appreciated."

In an eye without problems, the head of the optic nerve looks somewhat like a donut with a small cup in the center of it. This is where the nerve fibers are located. The hole, which is called the optic cup, should have a thick ring around it. Glaucoma is a condition where the fluid in the back of the eye is not able to drain the correct way, and as a result, an increase in pressure develops from excess fluid. In glaucoma, the increased pressure can lead to damaged nerve fibers and cause them to wear away. This in turn leaves a larger cup (or hole in the doughnut). This may be what you are experiencing, but you need to speak to your retinologist to know for sure. Also in diabetes, occasionally there are microvascular complications to the eyes called ischemic optic neuropathy. This refers to optic nerve damage, and again, could be caused by glaucoma or by other diabetes-related circulation impairments.
