"My blood sugar levels suddenly took a sharp turn upwards four
months ago without any change in diet or medicine. The Dr.
nor I have an explanation."

I was diagnosed with pancreatic cancer in May of 2007 and
had distal pancreatectomy in June of '07, followed by chemo
and radiation. After all the surgery and treatments were well
over, my readings were pretty good. Then about one year
later in October-November '08,it went completely out of control.
My Dr. now has me on Metformin twice a day(I was already on
Glipizide 2.5mg twice a day). I have only been on this for a few
days but see no difference. My morning readings are between
l60-200 and before dinner readings between 200-250. My
A1C has gone from 7.0 to 8.6 in these last four months. Do
you think I need insulin?

All oral medications require that your pancreas be making insulin in order to work. It appears that your ablility to make sufficent amounts of insulin to allow you to use your medications. Based on your past problems it would be wise for you to seek an appointment with an endocrinologist who could order a test to determine your insulin levels, and give you the definitive answer.
