FDA Approves Januvia for Type 2 Diabetes

October 17, 2006 (FDA) - The Food and Drug Administration (FDA) announced today the approval of Januvia (sitagliptin phosphate) Tablets, the first diabetes treatment approved in a new class of drugs known as DDP-4 inhibitors that enhances the body's own ability to lower elevated blood sugar.

FDA approved Januvia for use in addition to diet and exercise to improve blood sugar levels in patients with type 2 diabetes, alone or in combination with two other commonly prescribed oral diabetes medications, metformin or a PPAR (peroxisome proliferator-activated receptor gamma) agonist, when either of these drugs alone, along with diet and exercise, don't provide adequate blood sugar control.

"For the millions of Americans with type 2 diabetes, who continue to have inadequate blood sugar control, the approval of Januvia marks an important advance in the fight against diabetes," said Dr. Steven Galson, Director of FDA's Center for Drug Evaluation and Research. "We now have another new option that treats the disease in an entirely new way that can be added to existing treatment regimens to help patients gain more control over their blood sugar levels."

Type 2 diabetes is the most common form of the disease, accounting for about 90 percent to 95 percent of all diagnosed cases of diabetes (20.8 million in 2005). In type 2 diabetes, the body does not produce enough insulin or the cells ignore the insulin. Insulin is necessary to take sugar, the basic fuel for cells, from the blood into the cells. Over time, high blood sugar levels can increase the risk for serious complications, including heart disease, blindness, nerve damage, and kidney damage.

Januvia prolongs the activity of proteins that increase the release of insulin after blood sugar rises, such as after a meal. Januvia does this by blocking an enzyme (dipeptidyl peptidase IV or DPP-IV) which breaks down these proteins, leading to better blood sugar control.

Januvia was examined in a total of 2,719 patients with type 2 diabetes, in studies lasting from 12 weeks to more than a year. These studies demonstrated improved blood sugar control when Januvia was used alone or in patients not satisfactorily managed with metformin or a PPAR agonist.

The most common side effects in clinical studies were upper respiratory tract infection, sore throat, and diarrhea.

I have been a diabetic fo about 12 years in Canada we do counts differently mine is average 7.5 .I have used junuvia along with gluconorm and metformin and stopped due to itching and rashes,mainly on my back.I also tried levemir insulin which also has side effects,that are similar.My only problem is I stopped taking them at the same time,so I'm not sure which one was causing the problem.Any suggestions

I have been on Januva for 6 mos.I stopped 10 days ago.I had most all the above negative results in varying severe degrees and had weight gain of 10/15 lbs (probably fluid retention)and bad swelling of the throat area which resulted in 2 episodes of nearly fatal choking and jaw joint pain and ear ache also.Since stopping this medication my weight has dropped 7 lbs(no change in eating habits/activity).Most importantly my throat/esophogel swelling and jaw pain have disappeared.This medication is too risky to experiment/warrant use and I urge caution.My blood sugar has not materially changed and I must concentrate on diet/exercise.I have been Type II diabetic for 25 years.Using 15 unit Lantus in AM with 850 mg Metaformin with 10mg Glipizide AM/PM.

Iam a Home Health Care Worker.I have a client who was recently diagnosed, with type2 diabetes. His Dr. started him on Januvia. Since then his #s are good, but he is having, severe pain in his upper extremities. He is also a heart patient, with a history of congestive heart failure. So any pain he experiences, is a concern. Januvia needs to be reevaluated.

My Doctor has given me JANUVIA to help me manage my blood suger levels. It is working grest BUT I now have extreme itching all over my body, especially on the head. Has any one else reported this side effect?

Could this drug cause this itching??? It is driving me crazy!!!!

I have been taking Januvia (50)mg. for a couple of months. My doctor is increasing the dosage to (100) mg. I have been having large bowel movements and am constipated. I was admitted to the ER last night with severe pain (pulsating) just below my left ribcage. The ER doctor was not familair with Januvia. He mentioned possible pancreas problems which I am having further checked out this week. My Basphil was 2.4my Lipase was 310. Would Januvia have anything to do with this. I am the first patient my doctor has try this medication on. My blood sugar has been around 102-110 in the AM and I am combining it with Glucophase XR 750. Thank you. BB

I went off Actos 6 months ago because my doctor and I both noted my weight gain and he thought Actos might be to blame for part of that. I went on to Januvia. But Actos controlled my sugar very well -- it never went over 115. Now, it is up around 150. First time ever. So I guess you pick your poison. I've asked to go back on Actos and had blood tests today. Based on those results, I may go back on Actos. I think high sugar is more of a risk than 10 lbs around the abdomen area!

I have just begun taking Junevia for type 2 diabetis in place of Avandia. I am having sweating sessions and extreme weak spells. Is this normal? I had no trouble with Avandia but my Dr. said it could be dangerous. cccccccccccccccccccccccccccccccc

I took Januvia for about 3 to 4 weeks. I developed loss of my voice and little chest conjestion. I have been off of it for 2 weeks now have not gotten my voice back as of yet. I took a round of antibotic and no improvement. I also take novalog 70/30 and symlin. My doctor want me to get back on januvia, but I am afraid. Do you how long it will take to get my voice back? When I was on the januvia my reading were still running any where from 300 to 160, so it was sill very high.

I am on Day 4 of taking Januvia after being on Avandia for 3 years. Each day I've noticed that I am experiencing increasing joint pain in my jaw and face. not good. otherwise, my numbers have been good, even when eating stuff I shouldn't. but really, what's the point of low numbers if you're in pain and miserable?

Face it, drug companies don't want to come up with a cure for ANYTHING - too much profit to be made.

I have been taking Januvia for a couple months now and I have seen minimal reduction in BS but my foot problems are better. I also am having trouble with weight control and reduction. Will Byetta do a better job for me?

My doctor switched me from Glucotrol XL to Januvia. What a disaster that was. My #s were way high....was on it for 2 weeks. First I had headaches, then a severe sore throat, followed by upper respiratory infection. Needless to say, my doctor took me off Januvia and back on Glucotrol. I have no positive feedback for this drug.

My husband (age 82) was just diagnosed with Type 2 diabetes. His dr. started him on Januva approximately 6 weeks ago and he has so far lost 19 lbs! He was not overweight to begin with...6'tall....174 lbs. I know weight loss is one of the side effects, but I'm concerned about his weight loss. The dr. told him it would supress his appetite some, ...I can't understand why the dr put him on this med knowing my hubby wasn't overweight and shouldn't lose weight. I suppose we need to call the dr and see if he'll put him on something else. They should push this medication as a diet pill!

I have just started Januvia for my diabetes because Metformin caused me severe diarrhea with my other meds.

I have been a diabetic for many years. I am currently taking 2000mg metformin and amaryl4mg 2/day. My sugars are still on the high side before breakfast. Around 150-180. I walk occasionally but am good with my diet. Lots of berries and veggies. Avandia did not do well with me and Januvia does not bring my levels down either even with diet. I am hopeing I will not have to go on insulin soon. Any suggestions?

My insurance will not cover Januvia or Janumet without a prior authorization which I cannot get. Is there an alternative in the same sitagliptin family I could substitute for? This is really working well as I had just started on it! Please hurry with your response. Thankyou. Doug Grimm

I am a type II diabetic who, like so many, have been put onto different oral medicines. None of them have successfully lowered my early morning test readings. I usually have 150-170. About 3 months ago, my endocrinologist put me on Januvia along with Avandia. I was taking Avandia twice a day but my ankles were so swollen and hurtful I needed to change.
Now, although I do have some lower readings, I still get high blood sugar readings if I eat anything and sit. Even protein or power bars and sit and my blood sugar goes up to 160 and I get really sleepy.
I do better when I just eat a piece of chicken or a hamburger pattie with cheese and no buns.
I have had diabetes for years and tried all kinds of oral meds to avoid taking insulin shots. So far, its hit and miss and lots of aggrevation. I can't seem to lose enough weight to get rid of the stomach and my diabetes isn't in control either despite my increased physical activity and close control on my diet. Oh, and my back has been itching like crazy in the evenings.
Whats next?

Is Januvia in the same class of drugs as Byetta? I have recently been put on Byetta and it's very inconveniet to inject just before meals. Since we eat at various times, it makes the second injection inconveniet. I'm also on Metromin and Actos. Can Januvia be taken with those 2 meds? I will discuss this with my doctor next time I see him. Thank you.

I would like to know if this medicine is good for someone who has stage 4 Liver disease, and taking spirolactone, folic acid, prilosec, multi vitamin, iron folate? I have a liver biopsy, and it is on the last stage- I have had bleeding in the stomach, esophagus, ascites, portal hypertension, and high blood pressure. Dr.'s have prescribed glucophage, glyburide, and am presently taking daily doses( sliding scale) of insulin-Humulin R and want to know if this med. has really a proven record? What are the side effects? Why change to Januvia if someone is doing insulin( for the purpose of not upsetting the liver and not having the med. go through the liver)? The reason for doing insulin is to not upset the liver or have the liver process the medicine- glucophage or glyburide. Do you see what my question is? My doctor who is new to me is suggesting the Januvia, and I have to weigh this carefully.

IHAVE BEEN TAKING JANUVIA FOR ABOUT ONE MONTH AND HAVE HAD INCREASING CHEST PAIN, ARM PAIN SOMETIMES WITH NUMBNESS AND TINGLING AND EXTREME FATIGUE. I HAD NO ENERGY TO DO ANYTHING. VERY DIFFICULT TO EVEN GET OUT OF BED. I AM AN R.N. AND READ SOME OF THE SIDE EFFECTS ON THE WEB EVEN THOUGH THE INSERT SAYS THE SIDE EFFECTS ARE MINIMAL. I ALSO HAVE ANXIETY ATTACKS AND THIS HAS INCREASED ALSO. MY BLOOD SUGARS WERE GREAT 90-115 BUT IF I DID NOT EAT EVERY THREE TO FOUR HOURS MY LEVEL WOULD DROP INTO THE 70'S AND 80'S. I WAS EATING MORE JUST TO KEEP MY LEVELS UP. NOT GOOD FOR MY WEIGHT. I STOPPED THE MEDICINE APPROXIMATELY ONE WEEK AGO AND IMMEDIATELY FELT BETTER BUT IT IS TAKING A WHILE FOR SOME OF THE SIDE EFFECTS TO GO AWAY.

I was on a daily regimin of 2000 mg Metformin, and 15 mg glyburide. Due to liver enzymes showing up in blood, my regimen was changed the day before YESTERDAY to 100mg Januvia, 15 mg glyburide. (Metformin was cut out because my doctor feels it is damaging my liver.)

Yesterday morning on awaking, my blood glucose level was 150 mg/dl - not bad for me. I took my 100 mg Januvia and 10 mg glyburide. After lunch, which consisted of stir fry chicken breast and brocolli with a sweet sezchuan sauce, my blood had zoomed to 280 mg/dl !!!!

I was freaked out. There is no way i will allow that. and since my Januvia directions specifically say not to increase dosage, I decided to take a 500 mg Metformin and 10 mg glyburide to get my blood sugar down (i checked on the internet to see if you could safely use Metformin with Januvia.)

Two hours or so later, i took my daughter shopping and i began to feel the onset of low blood sugar... quickly nearing the point where thinking is difficult. i quickly returned to my car and tested my blood. It was 71 mg/dl! I freaked out again! I took a few glucose tablets to raise the blood sugar. when I felt better, we continued shopping.

We had pizza for dinner. did some arts and crafts with my daughter and watched a movie. I got her to bed, and fell asleep on the couch around 11 PM.

I awoke at 5 AM and tested my blood glucose level for the heck of it. 81 mg/dl !!!!

I double checked my testing strip lot number and calibrated my freestyle flash tester with a control solution and tested again. Yup, 83 mg/dl !!!!!


I'm trying not to be prematurely elated over these results with with combining 100 mg Januvia with 500 mg Metformin and the glyburide, but I'm cautiously exited. there seems to be some synergism going on here... and if it allows me to be controlled with the drastically reduced need for Metformin, i'll be a happy camper.

today will tell the story... i'll report the results tomorrow.

I have been on Januvia for 4 days now. I gained 30 pounds very quickly on Actos and Amaryl but my blood sugars were pretty well controlled. I was given false information that Januvia causes weight loss like Byetta. My blood sugar has been consistently high since I dropped Amaryl and started Januvia with my Metformin 1000mg twice a day. I was on Metformin 500mg twice a day with the Amaryl and pretty much controlled my sugars but was excited about new drug. Please let me know if I should continue or if it takes a while to get in my system. HELP!!!!!

I have been taking 10 mg Glipizide (5 mg AM, 5 mg PM) and 2000 mg Metformin (1,000 AM, 1000 PM) and have brought my sugar down from 250 to 300 to an average 150 (140 - 160) and have plateaued for the past six months. I can't seem top get it down any further. Will this new combo do better?

I started taking januvia two weeks ago along with amarilis, avandia and metmorfin, I stopped taking metmorfin several days ago perhaps with hopes of stopping the diarrhea. I have been taking emodium
to control the diarrhea, this was before I started taking januvia, the diarrhea persist and is
at tims uncontrolable with flow of fecal liquid
exhuding. I am taking two emodiums daily to somewhat control. Is there any other way to control the diarrhea? Januvia is working.

Whether it is coincidence or a side effect; since taking the drug, Januvia, I have been experiencing constipation. To a point of possibly experiencing a "near-birth" event!!!
Certainly not the diarreha reported.

Just started Januvia and within 2hrs my blood sugar was down in the one hundreds from 200s!!
Past 5days my numbers are close to normal and that is a wonderful improvement from the 400s!Been diabetic years type 2 and tried Insulin but the weight gain was horrible!! So far no side effects!

I am a type 2 diabetic on 70/30 novolog insulin. Few week ago my doctor gave me actoplut met in addition with my insulin to help lower my blood sugars. My ankles, feet were very swollen, I was achy all over my joint, my skin started to peel off my skin.Sure the pill help lower my blood sugar, and also made me eat so much more since my sugar drops, which causes weight gain. I am o.k. now since I stopped the med. Now my doctor started me on januvia, and after reading some of the side effects, I would not touch januvia. Will there ever be medication to treat this hirrible disease without the side effects giving more trouble than the diabetes itself. I feel like these doctors are using diabetics as guinea pigs. And that's sad. We have enough to deal with as it is.

I have been on Junuvia since December 21, 2006 and have experienced every side effect mentioned and even more. I have been able to reduce my doage of Levemir; however I have a stuffy nose, swollen ankles, chest pains, anorexia, headache. We have cut the dosage of Junuvia back to 50 mgm. and still have the side effects. Even had to be rushed to the Emergency Room and admitted to the hospital as I had severe nausea and vomiting with chest pains. (EKG & Nuclear Stress test was negative and pains subsided when I stopped taking the Junuvia.

I started taking Junuvia on Dec. 21, 2006. I have had all other meds for type II diabetes over the last forty years. Lantus--slowed my gasstric emptying and I did not have goog control of my glucose levels....a lot of hypoglycemic episodes in the mddle of the night. I am now taking glynase and Levemir Insulin. I started on the 100 mgm. Junuvia and had to decrease my Levemir from 40 units daily to 18 units daily. (Sometimes in divided doses--10 and 8 units. The Junuvia really brought my readings down. NOW, THE SIDE EFFECTS:::I had the stuffy nose, the sore throat, gastric disturbance--gas, bloating,etc. The chest pains (angina type) became so severe that I was rushed to the Emergency Room where they insisted I was having a heart attack=-but after complete nuclear scans and other tests, this was ruled out. (I spent two days in the DeBakey Heart Institute for complete testing and have followed up with my gastroenterologist and endocrinologist.) My dosage of Junuvia has been cut to 50 mgm. daily and I'm taking 8 units of Levemir in AM and 10 units PM. I still have some nausea and vomiting; however, the chest pains have ceased to be bothersome! I am a 82 y/o white male and am working through the side effects of Junuvia as the Levemir/Junuvia/Glynase combination seems to be working as I make adjustments to tolerate the present side effects and hope they will go away. (No studies utilizing Junuvia in patients has not been reported in the literature; however I could be an appropriate candidate!

I just started a dose of Jauvia in combination with metformin and over the counter vitamins.
I don't have the energy to exercise, I hope this will work for me.

How long before this medication starts working fully?. I've been taking 100mg once a day in conjunction to 1000mg of metformin twice a day for approximately 7 weeks, and still my blood sugar readings are rather high when I first get up in the morning.

I have been on this medicine for two weeks now and every since i started it i have had a cough and congestion. Its helped lower my suger but does any one know if the side effects last long ? Or is it to where your have it until you stop taken the pill.

Please let me know if I would benefit from taking Januvia. I am 77 yrs old, type 2 diabetic, taking Glucotrol XL medication and on insulin twice a day (Humalog 75/25). My Ha1C is 6.2% but my glucose goes up and down.

I am also on oxygen 24/7 so I am concerned about the possible side effect of respiratory infection. I also have CHF. Please advise. Thank you.

My husband has type 2 diabetes, he was recently
taken off of insulin, and put on januvia, he is also taking, actos once a day early of the morning,and metformin 2 times a day, he is suspose to take the januvia early,we are confused about how he should schedule his other medicine, to spread them out in a 24 hour period
please give us some advice,

I cannot swallow pills. Can Januvia be chewed or crushed or MUST it be swallowed whole? This is very important to me, does anyone know the answer to this question?

you all got it all wrong...januvia is good but not as good as Byetta. No where near. Januvia lowers a1cs but not not that good, no where near what a metformin or byetta or insulin does...and Januvia is weight neutral, and Byetta most patients lose weight, big difference for patients with type II diabetes. So if your overwieght and are on a oral, then byetta is your answer, if your slim and have type 2 diabetes then possibly januvia will work. Major questions thought, this is a dpp4 inhibitor that inhibits a important enzyme in the body, the main question is what else is it blocking in the process>

I have tried some samples of Januvia from my doctor for a few days; so far, the only side effects I have noticed have been a sometimes runny and/or stuffy nose, similar to an allergic response. I have been taking metformin before this, and had no noticeable side effects from it. I am considering returning to metformin, as I do not want the nasal problems on a daily basis. Also, the metformin is available in generic form, and is much less costly. I could not afford regular prescriptions of Januvia.

I've been playing with this blood sugar issue for a few years now. I just started the Januvia today. what I wonder is why blood sugar is treated as the problem. not as the symtom of the problem. logic would say that if body fat , calorie intake and exercise are contributers however not solely the source for high blood sugar. the problem must be something else.

I have been on Januvia for 7 days...WOW!!!What a difference in my numbers....I can't believe that it works so well.... And no side effects to speak of.. I am going to ask my dr. if I can eliminate the other drug I take..I have already lower the dosage from 45mgs. to 30mgs. (actos) Any info you can send me would be so helpful to me..Diabetes is a fight but you have made that fight so much easier to contain...Next we have to make the insurance companies move Januvia to a first tier drug so the co-pay isn't so expensive...THANKS AGAIN..........

I have been on Januvia for about a week. It has NOT lowered my blood sugar at all. My fasting BS are still high. I am using it as monotherapy. Anyone else using this drug? It cannot be used with insulin but can be used with Metformin. The pills cost $4.86 each and my insurance will not pay for it since it is still considered to be EXPERIMENTAL. I am going to take it a few more weeks and see what happens. It is available now from most of the endocrinologists.

After reading of the side effects above of the new diabetes drug, Januvia, I am afraid to take the medication! I am predisposed to upper respiratory tract infections & diarrhea, & I certainly don't need these things. My physician wants me to try Januvia along with metformin. I have tried Byetta which gave me really good control, however, it caused some sever reactions such as muscle cramps in my neck and arms, so I had to stop using it. Now I am hopeful that your new drug will work for me. I have medicare, but opt to take the Humana plan with drug coverage. I have been unable to find Januvia on their drug list and am wondering if it will be covered. Do you have any information on what medicare drug plans will pay on Januvia? Before I try this new therapy I will need to know this information, or at least know what cost will be if I have to pay for it myself. thanks. Nettie g. Bair.

I have been on all the oral meds at one time or another. I have tried Byetta, except it would lower my sugar after meals too much. Now, for the past six months, I have been on Levemir. I have had to increase to 93 units so far. I was doing well with it for the first couple of months. My sugars are 240's in the morning and somethins 160's. They are increasingly going up. Do these medications stop working after awhile? I have always had high morning sugars. I have gotten up in the middle of the night and they are up around 200 or better. Do OTC herbal remedies help at all? My sugar right now is 313 one hour after eating breakfast. I have been Type 2 for 15 years. My A1C is currently 7.2 down from 8. since months' ago.

my son has been in the honeymoon stage of diabetes for more than a year now. would Januvia benefit him. he is on insulin, but at times blood sugar runs so low he can't take his insulin. it's really frustrating for him....and me. thanks

This is all fine & great, but what about all of us with type I? I have had type I Diabetes for 29 years and it is getting more difficult to control. How about a cure? I guess if they cured type I, the pharmaceutical companies would lose so much money on strips, lancets, syringes and insulin. What was I thinking?

I have type 2 and take Lantus and Humalog. Can this drug be taken with insulin? I also take Byetta but am still having trouble with my morning sugars. I will be 70-100 at bedtime and wake up with 150-200. I am also having trouble losing weight since I've been on insulin. I have had a 40 pound weight gain in about 5 years

Am I correct in assuming that this medication is not for type 11 diabetics on insulin already? And if so, am I correct in understanding that if you are eventually put on insulin it is because your pancreas has either decreased or stopped producing insulin? Thanks for the info. However, I doubt belonging to the HMO that I belong to that people there will be able to have the new medication. They don't even allow you to have the pens ...that other insurances have...unless you can afford to pay the full price...a shame....but then that is the way of the insurance companies....

Thank God that they finally came up with a new
drug for type 2 diabetics. I was diagnosed about
5 years ago and it has been tough. My sugar is high, then low. I have been on almost everything
there is for diabetes. I take Actos, Amaryl, Metformin and Apidra Insulin when needed. I have
also taken Byetta and only God knows what else. My
weigh is 246lbs. I never weighted this much in my
life, also the steroids did not help my weigh either. I am excited about the new medicine.

I often wonder if its worth trying this new drug. Look, upper respiratory infections is one thing but diarrhea. come on. How can you go on your daily routine with having this. Let alone going out and getting stuck somewhere, where you have to stay near a bathroom all the time.....

Isn't Merck the maker of Vioxx, helloooo! I also heard one of the side effects are severe joint pain.

I am Type 2 insulin dependent, and I have found after being on Actos, metaformin, avandia and avandament, An Insulin cocktail of 70/30 really isn't so bad. Inconveneient, at time yes. Side effects, minimal.

I THINK THAT THIS MEDICINE HELP TO THE PATIENTS THAT NOT HAVE BEEN CONTROLED THIS DIABETES TYPE II AND AVOID DAMAGE AND COMPLICATIONS IN THIS PATIENTS. THE JANUVIA IS A GOOD OPTIONS.

I have been a diabetic for 8 years now and I have been on several diffrent type of insulin to lower it. When I heard about this I was happy. I can't wait to try this and get back to somewhat of a normal life again.
