Right now, I'm staring at my logbook trying to decide what changes I need to make and what else I want to try with all this. This past week has been a bit rocky so my averages are all over the place. Mostly, they're high. At least higher than I want them to be. So while I'm staring at my book, I'm trying to piece together this last week and figure out what is going on with these numbers.
My blood sugars both before and after breakfast are typically high (as in above 180). I tried adding more protein to my breakfast, but really didn't see any dramatic results. So I don't think that's the case here. I'd like to add Symlin in, but I don't have the means right now to get a sample (my CDE is 2 hours away and cannot mail one). So my next course of events is to lower my carb ratio to 1 unit to every 8 carbs (right now, it's 1:10). I'm frustrated at having to do this because calculating 1:10 is so easy, but 1:8 is tougher...and I'd rather make my diabetes as easy as possible.
I raised my evening Lantus to 14 units on Wednesday night, but I'm still seeing high morning blood sugars that I'm not liking at all. So tonight, I'm trying 16 units instead. (I know I mentioned raising it to 15 units last week, but I got a little scared and thought I'd go slower. Now I'm just frustrated with the 200's showing up.) I'm hoping that will start my mornings off better. If I can get those better controlled then my post-breakfast numbers will be easier to get into place as well. Because even though my post-prandials are high, they are lower than where I'm starting out usually.
I've been doing my morning Lantus earlier by about 4 to 5 hours and I've kept it at 13 units, which seems to have me okay throughout the afternoon. I've seen a few lows during those times, but nothing under 75, which is definitely something I can handle. But moving the Lantus up by those few hours seems to have me high in the late evenings, which I'm not sure how to correct unless I move my evening Lantus dose up as well (which I'm not even sure will do the trick). And since I'm not correcting any bedtime blood sugars under 250, I'm riding high for about nine hours of the day...definitely not good for my body or my averages.
With all these problem areas and highs, I'm wondering if I need to make even more changes. One big change running through my mind involves the pump. Since it automatically calculates my boluses, I'm tempted to use it for boluses and keep Lantus as my basal. So I'd be attached to the pump whenever I need a bolus, but not necessarily all the time. The less wear on the infusion site (as in not as much insulin going through constantly) might leave those areas less inclined to rashes, infections, and abnormal fat accumulation (which I really struggled with the last few months of pumping).
I'm considering trying this for the next three days to see how it does for me. I'm about to head on vacation, so I hate adding the pump back in right now but I also don't want to end up with terrible highs throughout my trip. Plus the ease of just using the pump for boluses will make it a little better since I can carry it in my purse and connect when necessary. I'm still not set on it, but it's definitely tossing around in my mind.
For now, I think I'm definitely going to move that evening Lantus injection up to about 8pm. It's roughly my dinner time, so I can just take those two injections together. It'll take a little getting used to and definitely complicates my life, but all the highs are complicating it too. So it's just a matter of seeing if it helps any. If not, I can always move it back again. Just wish I knew ahead of time.
