I was grocery shopping when the phone rang. I didn't feel the vibration in my coat pocket; the call went to voicemail. As I was checking out, I checked my phone. Two messages. One from my mom and one from the doctor's office.
"Hi, Miss Guerin. This is G from Dr. K's office. Your lab results are back. I'd like to talk to you. Please call me back at..."
My heart sank. I've heard those words too many times. At 21 years old, I shouldn't be hearing those words!!! If your lab results are fine, they don't call and ask for a return phone call. They tell you that everything is fine or that the results are in the mail. You're not important if you're healthy.

I took Charlie to his endo appointment yesterday. It was the first time we didn't come with an unraveling scroll full of questions. I had no questions because I believe they have no answers.
It's not that Charlie is doing so poorly. His latest A1c of 8.2 was his best ever. But his numbers have been rough since school started and patterns have become difficult to spot.
A pair of medical students entered the room with big smiles and lots of questions. I couldn't help sounding very matter of fact and slightly defeatist.
"Yes, we're counting carbs correctly. Reading labels, subtracting fiber, all that fun stuff," I said.
"Yes, we have the Calorie King book."

I wish a pizza party could be just that. A party with pizza. For a parent of a child with diabetes, it's no party at all. It's a highly strategic military operation.
Charlie went to a birthday party on Sunday where the kids made their own pizza. I would have preferred a make-your-own grilled halibut with asparagus party. But it was not to be. (READ MORE)

"Take two tablets every four to six hours, up to four times daily."
These are the standard dosing instructions we see on drugstore bottles of aspirin, acetominophen, and ibuprofen. "Take two [at the time]" is such a common instruction for medications that it's almost become a cultural joke. But if you go over to the drug manufacturer's site, or to a site like drugs.com, you'll find another story that may cause you to question the first. That story is the drug's activity curve.

Tags: me split lantus doses why

Tuesday night, I switched my Lantus over to a once-per-day shot instead of the split doses I'd been using previously. I started with 22 units of Lantus at 8pm. I knew I'd see some highs, but I didn't want to risk an unexpected night low after making the first switch.
And I did see some elevation. I ran mostly in the 180-290 range all through Wednesday, which wasn't as high as I was expecting really. I made sure to consider food in the highs, along with the usual post-breakfast spike. And I was confident that a few more units of Lantus might do the trick (or at least get me closer).

It's the end of my logging week again, so I'm sitting down to examine the logbook that I hold so dearly close to my diabetes management's heart. The time that I set apart for this goes something like this. First, I update the logbook with my most recent numbers, Lantus doses, and any important comments including new prescriptions, strange food choices, or exercise. Next, I tally daily averages as well as averages for time of day. After that, I analyze those averages compared to the last weeks averages and look for any resounding patterns that might need changing.

The problem that I have with Lantus is that one dose just doesn't cut it for me. It claims a 24 hour lifespan, but honestly...it's probably more like 20 hours on a good day. Which means that my blood sugars start spiking towards the end of that injection.
That's exactly why I split my doses however many years ago. It worked, dropping my A1c like a lead balloon. And now that my schedule nearly forced me into either the insane asylum or a once-per-day dose, I'm struggling to find an appropriate time for my Lantus injection.
An evening shot wasn't working because I was having to take such a bulk of insulin that I was dropping way below my safety point by morning. And now I've switched to an early afternoon injection, but I'm afraid that looks like it's bombing as well.

I'm kind of amazed at how such a small dose of Metformin can help my blood sugars in such a great way. I increased my dose to 500 mg on February 14th, but after a few days of pretty severe nausea, I decided to give myself a break to focus on a research paper and work. Yesterday, the 23rd, I started back on the Metformin at 250 mg.
But in those short five days off, my blood sugars started skyrocketing again. I was quickly seeing numbers in the 400 range and running consistently above 200. Nothing else changed except the Metformin.
I expect lower numbers by tomorrow even with the small 250 mg dose. I'm not sure how long I plan to keep it at the 250 mg since I do want to get to the max dose quickly, but I also can't deal with the side effects of this drug. I do not do well with nausea. It stops me cold in my tracks and prevents me from doing anything more than curling on the sofa and wanting to die.
