The following promotion is intended for participants in the United States only and shall be construed and evaluated according to the laws of the United States. Do not proceed in this promotion if you are not a legal resident of the United States.
NO PURCHASE OR MEMBERSHIP NECESSARY TO ENTER OR WIN. JOINING dLIFE WILL NOT IMPROVE YOUR CHANCES OF WINNING. For a free game card, mail a self-addressed stamped envelope to dLife Diabetes IQ Game Piece, P.O. Box 8830, Westport, CT 06888. Game card entry requests must be received by December 12, 2008. Game ends December 31, 2008.

PRIZES: ONE (1) GRAND PRIZE: Two-day/two-night trip for four to DisneyWorld. Prize includes roundtrip coach airfare for four from the major airport nearest winner’s home, accommodations for four (two bedroom kidsuite) and four Disney World tickets (ARV: $4,000). 10 First Prizes: Apple iPod (ARV $249). 100 Second Prizes: dLife Cooking DVD (ARV $15). 500,000 Third Prizes: Blood Glucose Monitor (ARV $50). Odds of winning are dependent on the number of eligible entries received. Actual value of grand prize will vary depending on winner’s city of departure. If actual value turns out to be less than the stated ARV, the difference will not be awarded in cash. All expenses not specifically included, including but not limited to any taxes or expenses as set forth, are the sole responsibility of the winner. Selected airline is at the sole discretion of dLife. If any element of the grand prize becomes unavailable, dLife reserves the right to substitute another element of equal or greater value. No prize or cash substitutions permitted by winner. Reservations are accepted on a space available basis and certain restrictions and blackout dates apply.
TO CLAIM YOUR PRIZE: Grand, first and second prize winners must mail in their game card, along with their name, address and phone number printed on a 3 x 5 card, to dLife Diabetes IQ Game Prize Claim, P.O. Box 8830, Westport, CT 06888. Grand and First Prize Winners must send their winning game cards via registered mail. All prize claims must be postmarked by January 9, 2009. dLife is not responsible for lost, late, damaged, misdirected, incomplete, illegible or postage due mail. If the Grand Prize trip is not claimed, it will be awarded in a random drawing from all valid game cards submitted for verification. Prize drawing will be held January 16, 2009. Other unclaimed prizes will not be awarded.
ENTRIES: All prize claims are subject to verification and become the sole property of dLife and will not be returned. Any game card received that is tampered with, illegible, mutilated, mechanically reproduced, contains printing or other errors, or was obtained where prohibited by law is automatically void. Any attempt to forge promotion materials in any way will be prosecuted to the fullest extent of the law.
GENERAL CONDITIONS: Sweepstakes open to legal residents of the 50 U. S. & DC, 18 years or older. Employees of dLife and their immediate families, it’s agencies and healthcare professionals are not eligible. Only one winner per household. Grand prize winner must sign and return an affidavit of eligibility, including proof of age, and a liability/publicity release within 14 days of first attempted notification. Tennessee residents need not sign publicity release. Winner’s travel companions must sign a liability release prior to travel. Winner and travel companions must travel on the same itinerary. Failure to provide signed documents will result in forfeiture of prizes. Potential winners forfeit prize if not claimed within ten (10) days of first attempted notification, whether notification is by e-mail, regular mail or telephone, or, if after reasonable attempts are made, notification is unsuccessful. Void in Puerto Rico and where prohibited. Entrants grant to dLife the right to use and publish their likeness, proper name and state of residence in all media in connection with this sweepstakes. Entrants release dLife and Affiliate Entities from any liability or injuries incurred in connection with participation in this sweepstakes or acceptance or use of any prize. dLife reserves the right to terminate or modify this promotion if any causes beyond dLife’s control impairs its integrity or fairness, in which case the grand prize will be awarded in a random drawing from all entries received as of the date of termination. dLife is not responsible for any problems or technical malfunctions of any telephone, network or lines, computer online systems, servers or providers, computer problems or traffic congestion on the Internet or at any website, or any combination thereof including any injury or damage to entrant’s or any other person’s computer related to or resulting from playing or downloading any materials in the Sweepstakes. dLife is not responsible for phone line failure and/or malfunction.
